Job summary

Island Home Care, part of LHC Group and affiliated with Optum, seeks an experienced Registered Nurse to deliver and direct nursing care in patients' homes, coordinating the total plan of care with physicians and the health care team.

The role requires an unrestricted RN license in the state, a valid driver's license, and strong communication and organizational skills to ensure high-quality patient outcomes across diverse home settings.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed nurses, provides instruction, and assigns tasks according to state and federal regulations.
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Documents patient visits per policy and payer requirements, and syncs timely per policy.
