Qualifications

• Active RN license in PA
• Minimum 1 year RN home health experience
• Valid driver’s license and ability to drive 20% of workday

Responsibilities

• Deliver skilled, patient-centered care in patients' homes
• Perform patient assessments, medication management, and care planning
• Educate and empower patients and families on disease management and treatment plans
• Maintain thorough EMR documentation and comply with policies
• Travel to patient homes within assigned territory
