Elara Caring is seeking a Registered Nurse for after-hours on-call to support home health patients in their own homes. The role emphasizes autonomous, compassionate nursing care delivered in a home setting, with travel up to 50% to visit patients where they live and recover.

The ideal candidate holds an active RN license, has at least 1 year of clinical nursing experience, and demonstrates strong communication skills to coordinate with supervisors and physicians.

Formación

• Graduate of an accredited nursing program.
• 1+ year of nursing clinical experience.
• Current State license as a Registered Nurse.
• Ability to travel approximately 50%

Responsabilidades

• Assesses assigned home health care patients to identify physical, psychosocial, and environmental needs.
• Makes initial evaluation and re-evaluates nursing needs during each visit.
• Communicates significant findings to clinical supervisor and physician.
• Counsels and implements the plan for patient safety using family and community resources.
• Participates in implementing the plan of care to ensure quality and outcomes.
• Verifies the Plan of Care prior to each visit and provides care per physician orders.
