Job summary

Amedisys Home Health is seeking an Admissions Registered Nurse to start-of-care assessments and implement skilled nursing plans for patients at home. This role emphasizes comprehensive evaluation, timely OASIS completion, and coordination with the care team.

Qualifications

Current RN license, state(s) you are assigned to work.
Current CPR certification.
Valid driver's license, reliable transportation and liability insurance.

Responsibilities

Performs start-of-care comprehensive assessment and develops the plan of care.
Completes OASIS assessments timely and accurately.
Provides skilled nursing care per plan of care and agency policies.
Maintains continuity of patient care through effective communication with staff, physicians, families, and resources.
Initiates appropriate referrals when indicated.
Promotes patient health through education and rehabilitative measures.
