Adara Home Health, Inc. is seeking an RN Admission Nurse to ensure client care aligns with policies and procedures. The role travels to client homes for 1:1 care, reports to the Clinical Supervisor, and supports referrals from various sources.

The position is full-time with Monday–Friday schedule and an on-call rotation, covering Owatonna and surrounding cities from the Mankato, MN office. Competitive benefits apply.

Formación

• Currently licensed as an RN in Minnesota.
• At least two years of experience in health care facility or home care.
• Strong organizational skills and ability to work independently.
• Excellent written and oral communication skills.
• Basic computer skills; Word and Excel a plus.
• Understanding of Medicare certification and OASIS documentation.
• Valid driver's license and access to a vehicle for travel.

Responsabilidades

• Perform initial and ongoing assessment of clients and implement care plans.
• Provide total client care including medications and treatments as ordered by physician.
• Coordinate client care with family, other caregivers, and community resources.
• Maintain client records and ensure confidentiality and infection control.
