Job summary

Adara Home Health in Minnesota is seeking a Clinical Admissions Nurse to ensure client care adheres to company policies and fosters strong relationships with referral sources. This role reports to the Clinical Supervisor and involves travel to client homes for 1:1 care.

The position is full-time with an on-call rotation, serving Waconia and surrounding cities while coordinating client records, care plans, and education to support optimal outcomes.

Qualifications

• Must be currently licensed as an RN in Minnesota.
• Minimum 2 years healthcare facility or home care experience.
• Strong organizational and independent working skills.
• Ability to make appropriate clinical judgments and communicate effectively.

Responsibilities

• Perform initial and periodic client assessments and update care plans.
• Provide total client care including medications and education per physician orders.
• Coordinate client records and care with family, other caregivers, and community resources.
• Maintain confidentiality and follow infection control policies.
