Job summary

Phoenix Home Care & Hospice is seeking a dedicated RN to provide skilled visits to patients in the Kansas City area. This full-time role includes on-call overnights and weekend coverage on a 7 days on / 7 days off rotation, with a mission-driven team focused on compassionate care.

The ideal candidate holds an active RN license in the state, can work in a fast-paced setting, and demonstrates strong organizational and customer service skills.

Qualifications

• Active RN Nursing License in the state.
• Ability to work in a fast-paced, dynamic environment.
• Excellent organizational and self-management skills.
• Strong and compassionate customer service skills.
• Valid driver's license and reliable transportation.

Responsibilities

• Skilled visits to patients based on their needs.
• Daily patient interaction, ensuring comfort and support.
• Education on health conditions including CHF, COPD, Diabetes.
• Wound care and assessments.
• Medication management and administration.
• Change dressings, bandages, and other contraptions as prescribed.
• Documentation of patient's condition, reporting any significant changes to the physician.
• Collaboration with other healthcare professionals, including Physical, Speech, and Occupational Therapists.

• Salary Range: $80,000-$95,000

Requirements

Active RN Nursing License in the state.

Ability to work in a fast-paced, dynamic environment.
