Responsibilities

• Skilled visits to patients based on their needs.
• Daily patient interaction, ensuring comfort and support.
• Education on health conditions including CHF, COPD, and Diabetes.
• Wound care and assessments.
• Medication management and administration.
• Change dressings, bandages and other contraptions as prescribed.
• Documentation of patient’s condition, reporting significant changes to the physician.
• Collaboration with Physical, Speech, and Occupational Therapists.

Requirements

Active RN Nursing License in the state.

Ability to work in a fast-paced, dynamic environment.

Job Duties

Skilled Visits to patients based on their needs.

Daily patient interaction, ensuring comfort and support.

Education on health conditions including, but not limited to, CHF, COPD, Diabetes.

Wound care and assessments.

Medication management and administration.

Change dressings, bandages, and other contraptions as prescribed.

Documentation of patient’s condition, reporting any significant changes to the physician.

Collaboration with other healthcare professionals, including Physical, Speech, and Occupational Therapists.
