Job summary

Ohana Pacific Health is seeking a dedicated Home Health Registered Nurse to provide skilled nursing care in patients' homes in Hawaii. You will work closely with patients, families, physicians, and an interdisciplinary team, delivering comprehensive assessments and education to promote wellness and independence.

Requirements include an accredited nursing degree, current Hawaii RN license, BLS certification, and at least two years of hospital experience; Medicare home health experience is

Qualifications

Graduate of an accredited school of nursing.
Current Hawaii RN license required.
BLS Certification required.
At least two years' hospital experience preferred.
Medicare Home Health Agency experience preferred.

Responsibilities

Provide skilled nursing care to homebound patients in their home setting.
Lead and collaborate with an interdisciplinary team to achieve patient goals.
Complete comprehensive admission history and physical assessment.
Educate patients and families on health and wellness at home.
Promote effective communication among care disciplines.
