Job summary

Ohana Pacific Health is seeking a Home Health Registered Nurse to deliver skilled nursing care at patients' homes, coordinating with physicians, families, and the care team. A BSN is required or ADN with substantial home health experience.

The role emphasizes leadership of an inter-disciplinary team, comprehensive admission assessments, and patient education to promote health and wellness in the home setting. Relocation assistance may be available.

Qualifications

• Bachelor of Science in Nursing (BSN) required.
• Associate Degree in Nursing (ADN) may be considered in lieu of BSN with minimum 6 years direct patient care in home health or related setting.

Responsibilities

• Provide skilled nursing care in patients' homes in collaboration with the interdisciplinary team.
• Lead an inter-disciplinary team to achieve individual patient goals.
• Complete comprehensive admission history and physicals and educate patients and families on health and wellness at home.
