Job summary

CenterWell Home Health, a Humana company, is seeking a Home Health Registered Nurse to provide skilled nursing care in patients' homes, develop care plans with clinicians, and monitor progress.

The role includes supervising Home Health Aides, educating patients and families, and coordinating with physicians and the Clinical Manager. Eligible candidates have RN licensure, CPR, and transportation.

Qualifications

• Med-surg, ICU, ER, or acute care experience.
• Current and unrestricted Registered Nurse license.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Valid driver's license and auto insurance with reliable transportation.

Responsibilities

• Provide skilled nursing care in patients' homes.
• Develop and implement individualized care plans in collaboration with physicians.
• Monitor patients' conditions and report changes.
• Educate patients and families on disease management, medication, and treatment options.
• Oversee Home Health Aides and coordinate documentation.
