Job summary

Caretenders, a part of the LHC Group and Optum family, seeks an experienced Registered Nurse for home health. You will coordinate the total plan of care with physicians and care team, ensuring compliance with laws and agency policies, while supporting patients and families in their homes.

Responsibilities include developing and overseeing care plans, supervising home health aides, and ensuring high-quality clinical services, with emphasis on patient safety and effective communication.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Must have a valid driver's license and access to a dependable vehicle or public transportation.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed nurses, provides instruction, and assigns tasks according to regulations.
• Provides required supervisory visits.
• Documents patient visits and prepares comprehensive assessments (OASIS) as required.
• Communicates with physicians, patients, families, and other team members to ensure quality care.
