Job summary

Caretenders, a part of LHC Group and member of the Optum family, is hiring a Registered Nurse for Home Health in Millbury, MA. You will provide nursing care in patients' homes as prescribed by physicians and coordinate the total plan of care with the care team to ensure continuity and quality.

The position offers a $10,000 sign-on bonus for qualified full-time external hires, a flexible weekday schedule, and comprehensive benefits supporting health, well-being, and work-life balance.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and reliable vehicle or access to transportation.
• CPR certification preferred or able to obtain within 90 days.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with physician and health team.
• Supervises care by home health aides and LPN/LVNs; provides instruction and assigns tasks per regulations.
• Completes assessments (OASIS), documents visits per policy, and ensures continuity of care.
