Job summary

Qualifications

• RN license in WI required
• At least 1 year of nursing experience preferred in home health
• CPR certification required
• Knowledge of state home health regulations and clinical proficiency

Responsibilities

• Provide home-based care to patients with illnesses, injuries and chronic diseases
• Oversee LPNs, CNAs and aides delivering patient care
• Conduct patient assessments, document progress and communicate with physicians
• Educate patients on plan of care, medications, therapy, diet and exercise
• Coach family members on the care plan and home safety
• Coordinate discharge from services and ensure goals are met
