Job summary

BAYADA Home Health Care is seeking a Full Time Admissions Registered Nurse (RN) to join our home health visit team in Devon, Pennsylvania. You will cover Delaware County and parts of West and South Philadelphia, providing 1:1 skilled nursing care to help clients remain at home.

Responsibilities include comprehensive home assessments, medication reconciliation, wound care, and teaching with strong independent decision-making in dynamic home environments.

Qualifications

• Current registered nurse in Pennsylvania with an active license.
• Graduation from an accredited nursing program as indicated by diploma.
• At least one year of adult/geriatric clinical experience in acute care, home health, or rehab settings; prior home health is required.

Responsibilities

• Perform initial home health admission assessments and triage.
• Manage medication reconciliation, teaching, and safety monitoring.
• Conduct comprehensive wound assessment, care, and documentation.
• Utilize head-to-toe assessments to identify changes in patient condition.
