Job summary

The Judge Group is seeking a dedicated Home Health Registered Nurse to provide skilled nursing care in the homes of patients across Cullman, Blount, Winston, and Walker Counties.

Responsibilities include comprehensive assessments, wound care, IV therapy, and patient education. A valid Alabama RN license, driver's license, and strong independent work ethic are required.

Responsibilities

• Conduct comprehensive nursing assessments and develop individualized plans of care.
• Provide skilled nursing services per physician orders and agency guidelines.
• Deliver and manage wound care treatments and IV therapy.
• Monitor patient progress and communicate changes to physicians and care team.
• Educate patients and caregivers on disease management, medications, and self-care.
• Coordinate care with physicians and other health professionals to ensure optimal outcomes.
• Maintain accurate, timely clinical documentation.
• Support patients in achieving health goals while promoting independence at home.

Job description

Coverage Area: Cullman, Blount, Winston, and Walker Counties
Schedule: Full-Time | 40 Hours per Week
Shift: Monday–Friday, 8:00 AM – 4:30 PM
Productivity Expectation: 30 points per week
On‑Call/Weekend Requirement: As requested by the agency
Mileage Reimbursement: Yes, reimbursed between patient visits (from first patient to last patient)
Rate: $45.00 per hour

Position Overview

We are seeking a dedicated and experienced Home Health Registered Nurse (RN) to provide skilled nursing care to patients in their homes throughout Cullman, Blount, Winston, and Walker Counties. This role is ideal for a compassionate clinician who thrives in an independent setting while delivering exceptional patient-centered care.

Key Responsibilities

• Conduct comprehensive nursing assessments and develop individualized plans of care.
• Provide skilled nursing services in accordance with physician orders and agency guidelines.
• Deliver and manage wound care treatments and IV therapy services.
• Monitor patient progress and communicate changes in condition to physicians and care team members.
• Educate patients and caregivers on disease management, medications, treatments, and self-care techniques.
• Coordinate care with physicians, therapists, and other healthcare professionals to ensure optimal patient outcomes.
• Maintain accurate, timely, and compliant clinical documentation.
• Support patients in achieving their health goals while promoting independence and safety in the home environment.

Required Qualifications

• Current Alabama Registered Nurse (RN) License in good standing.
• Experience with wound care and IV therapy/care.
• Strong clinical assessment and critical-thinking skills.
• Excellent communication and patient education abilities.
• Valid driver's license and reliable transportation.

Preferred Qualifications

• Experience in home health nursing.
• Proficiency with Homecare Homebase (HCHB).
• Strong organizational and time-management skills.
• Ability to work independently and effectively manage a patient caseload.
