Job summary

Cullman Regional Medical Center, Inc. in Cullman, AL, seeks a full-time Home Health Registered Nurse to provide patient-centered care in the home setting. Day shift.

As a patient advocate and case manager, you will assess needs, develop plans of care, and coordinate services with physicians and community partners. The ideal candidate holds an Alabama RN license, an associate degree in nursing, and at least two years in medsurg/ICU/ER. Home care or hospice experience is preferred.

Qualifications

• Associate's degree in nursing and Alabama RN license.
• Minimum 2 years hospital medsurg/ICU/ER experience; home care/hospice preferred.
• Strong critical thinking and organizational skills; able to work independently; flexible scheduling; prioritize patient needs.

Responsibilities

• Evaluate and admit patients to home care; develop plan of care and provide care to patient according to plan.
• Act as case manager making necessary adjustments in the patient's plan of care related to symptom changes and changes in care needs.
• Review and plan patient care and discharge during scheduled case conference meeting and document accordingly.
• Act as liaison for patient/caregiver with MD and community healthcare partners.
• Strive to lower cost of care through managing mileage, supplies and visit scheduling.
• Completes all documentation related to patient contact, assessments, SOC, and recerts.
• Resumption of care and discharge according to policy.
• Maintain understanding of current home care regulatory guidelines.
• Provide on-call coverage for both home care and hospice as assigned.
• Demonstrate and encourage team behavior and exceptional patient/guest experiences.
• Uphold and promote patient safety and quality.

Qualifications

Education: Associate's degree in nursing and currently licensed by the state of Alabama required.

Experience

Minimal 2 years' experience in a hospital setting working in medsurg, critical care or ER. Previous home care or hospice experience preferred.
