Job summary

Adoration Health is seeking a passionate Home Health Registered Nurse to join our team in Clanton, AL. This PRN role focuses on 1:1 patient care, delivering expert nursing support in patients’ homes and coordinating care with MD orders and family members.

You will assess needs, create care plans, administer medications, and collaborate with an interdisciplinary team to ensure high-quality outcomes. CPR certification and a valid state license are required.

Qualifications

• Registered Nurse degree from an accredited nursing program with current unrestricted license in the state of practice and residence.
• One year of nursing practice in a patient care setting; home health or geriatrics preferred.
• Valid driver's license, acceptable driving record, and proof of car insurance per Adoration policy.
• New grads may be considered in select markets based on program availability.
• Current CPR certification.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create home health care plans aligned with MD orders and patient goals.
• Direct nursing care: administer medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with an interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by the local branch.

• Registered Nursing Degree (Associate or Bachelor) from an accredited college of nursing with current unrestricted registration and license in the state of practice and in the state of residence is required
• One year nursing practice in a patient care setting required; and home health, geriatrics or other related settings preferred
• Valid driver's license, acceptable driving record, and proof of car insurance in accordance with Adoration policy
• New nursing graduates may be considered in select markets based on program availability
• Current CPR certification
