Job summary

UnityPoint Health’s Home Health team is seeking a Home Health RN to deliver one-on-one care with total autonomy over the role. The position features a flexible schedule, making it possible for patients to receive care in their homes, with oversight of the home health plan of care under physician direction.

The Home Care RN delivers patient care aligned with the plan of care, adhering to clinical best practices to achieve high-quality outcomes and superior patient experience.

Qualifications

• Graduate of State Board approved program for Registered Nurses.
• Valid RN license in the state where providing care.
• Valid driver’s license with automobile insurance.
• CPR/BLS Healthcare Provider Certification.
• Minimum of 1 year nursing experience.

Responsibilities

• Makes visits to the homes of patients requiring home health nursing services and assumes responsibility for ongoing interdisciplinary assessment of the patient.
• Performs initial and comprehensive assessments on all patients per agency policies and procedures.
• Performs nursing assessments at each visit per agency policies and procedures.
• Ensures quality and safe delivery of services to patients according to the care plan.
• Uses equipment and supplies safely, effectively, and efficiently.

Shift: 8:00 AM - 5:00 PM(4 days/week + weekend/on-call rotation)

The Home Care RN uses clinical nursing knowledge, physical assessment, teaching and procedural skills to deliver high quality patient care in the patient’s place of residence. The Home Care RN develops and oversees the home health plan of care under the direction of the physician and ensure appropriate interdisciplinary involvement to ensure the best outcome for each patient. The Home Care RN delivers patient care directed by the physician as established in the home health plan of care that is consistent with clinical best practices and results in high quality, improved outcomes and exceptional patient experience.

Qualifications

Education:

• Graduate of State Board approved program for Registered Nurses

Licensure/Certifciations:

• Valid license as Registered Nurse in state where providing care.
• Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
• Mandatory Reporter: Child & Dependent Adult Abuse.
• CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.

Minimum Experience:

• One year nursing experience caring for similar patient population as to be assigned.

Other:

• Use of usual and customary equipment used to perform essential functions of the position.
