Job summary

UnityPoint Health in Iowa seeks an experienced RN Case Manager to support UnityPoint at Home. This 1.0 FTE role offers 40 hours weekly, 8:00am–4:30pm, with some call, rotating weekends and holidays. A $10,000 sign-on bonus is included.

You will provide one-on-one in-home patient care with total autonomy over the care plan from admission to discharge, and develop and oversee the home health plan under physician direction to ensure optimal outcomes and patient experience.

Qualifications

• RN, licensed in state(s) where providing care.
• Minimum one year of nursing experience with a similar patient population.
• Valid driver's license and automobile insurance as required.
• Mandatory Reporter: Child & Dependent Adult Abuse.
• PCC course within 12 months of hire; annual competency validation.
• CPR/BLS certification with re-certification.
• Basic knowledge of commonly used medications, administration, actions and side effects.
• Competency in a range of nursing skills; ability to perform without supervision (e.g., NPWT, complex wound care, IV therapy, catheter care, ostomy care, chronic disease management, teaching).
