Job summary

CommonSpirit Health at Home is seeking a Home Health RN to deliver compassionate, high-quality nursing care in patients’ homes. You will assess, plan, and implement individualized care, ensuring patient independence and well-being with a strong emphasis on education and family support.

You will collaborate with a multidisciplinary team, mentor other clinicians, and utilize technology to optimize care delivery while maintaining thorough documentation.

Qualifications

• Graduate of an accredited nursing program with an active RN license in Kentucky.
• Minimum of 1 year of nursing experience (home health experience is a plus).
• Exceptional critical thinking, proven clinical expertise, strong case management abilities, and unwavering commitment to patient-centered care.
• Current CPR/BLS certification.
• Valid driver’s license and reliable transportation.

Responsibilities

• Be a clinical leader delivering high-quality, personalized nursing care in the patient’s home environment.
• Craft individualized care plans and adjust them to promote independence and well-being.
• Collaborate with a multidisciplinary team to ensure holistic care.
• Educate and empower patients and families with essential teaching and support.
