Interim HealthCare, the nation’s first home care company, is hiring a Home Health Registered Nurse (RN) in Hudson, WI and surrounding area. This full-time role offers one-on-one patient care in home settings, flexibility and opportunities to build lasting patient relationships.

The RN will assess, educate and support patients and families while coordinating with physicians and a team of LPNs/CNAs. A Wisconsin RN license, CPR and 1 year of nursing experience are required.

Qualifications

• RN licensure in Wisconsin (active)
• Minimum 1 year nursing experience, preferably in home health or hospice
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency and strong communication skills

Responsibilities

• Provide home-based care to patients after hospital discharge or in chronic conditions
• Oversee LPNs, CNAs and aides delivering patient care
• Perform patient assessments, document progress, report changes to physician
• Educate patients on plan of care, medications, therapy, diet and exercise
• Coach family members on care plan, medications and home safety
• Coordinate discharge from services to ensure smooth transition

As a Home Health Registered Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with illnesses, injuries and chronic diseases - often after discharge from a hospital, rehab or nursing facility
• Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care
• Examine patients, do assessments, document progress and report changes to their physician
• Educate patients on their plan of care, prescribed medication, therapy, diet and exercise
• Coach family members on the patient's plan of care, medication and home safety
• Ensure goals are met and coordinate discharge from services

A few must-haves for Home Health Registered Nurses:

• Graduate of an accredited nursing program and active RN license in Wisconsin
• Minimum of 1 year of nursing experience, ideally in hospice or home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing specialty, strong communication skills and compassion
