Interim HealthCare, the nation’s first home care company, is hiring a Hospice Registered Nurse (RN) to cover Hudson, WI and surrounding area. You will provide compassionate, home-based care to patients with six months or less to live, collaborating with an interdisciplinary team to implement care plans.

Qualified candidates have an active Wisconsin RN license, 1 year in nursing (preferably hospice/home health), CPR, and knowledge of home health regulations.

Qualifications

• Graduate of an accredited nursing program and active RN license in Wisconsin.
• Minimum of 1 year of nursing experience, ideally in hospice or home healthcare.
• CPR certification.
• Knowledge of state and federal home health regulations.
• Clinical proficiency in nursing and strong communication and compassion.

Responsibilities

• Provide home-based care to patients with six months or less to live, focused on quality of life.
• Work with an interdisciplinary team to carry out the patient’s plan of care.
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, and administer treatments.
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies.
• Educate family on prognosis, changes in care plan, and hospice goals.

As a Hospice Registered Nurse, here’s a big-picture view of what you’ll do:

• Provide home-based care to patients with six months or less to live, focused on quality of life
• Work as part of an interdisciplinary team comprised of a physician, LPN, CNA, HHA, social worker, chaplain and volunteers to carry out the patient’s plan of care
• Perform patient assessments, check vitals, evaluate breathing, monitor pain, administer prescribed medication and treatments
• Ensure accessibility of medical equipment, prescribed oxygen, medication and other supplies
• Educate family on the patient’s prognosis, changes in care plan, medication and hospice goals

A few must-haves for Hospice Registered Nurses:
