Interim HealthCare in Green Bay, WI is hiring a Home Health Registered Nurse (RN) to provide in-home care, coordinate with physicians and supervise LPNs and CNAs in the Green Bay area.

The role offers a base salary of $75,000–$85,000 with bonuses, mileage reimbursement and a comprehensive benefits package including medical, dental, vision, 401(k) and PTO. Pleasant, autonomous work with flexible assignments and ongoing professional growth.

Qualifications

• Active RN license in Wisconsin.
• Minimum 1 year nursing experience, preferably in home health or hospice.
• CPR certification required.
• Strong communication skills and compassion for patients.

Responsibilities

• Provide home-based care to patients with illnesses or injuries in Green Bay and surrounding area.
• Work with a team of physicians and oversee LPNs, CNAs and aides providing patient care.
• Examine patients, perform assessments, document progress and report changes to physicians.
• Educate patients on plan of care, medications, therapy, diet and exercise.
• Coach family members on plan of care, medication and home safety.
• Ensure goals are met and coordinate discharge from services.
