Rosaires Care is seeking a Primary Care Nurse to provide in-home, field-based RN services for adults and geriatric patients. You will assess, plan, and coordinate individualized care plans, manage medications, and deliver skilled nursing interventions in patients' homes.

Collaborating with physicians, therapists, and social workers, you will support safe transitions of care and coordinate treatment across a community-based service area.

Primary Care Nurse (Field-Based RN)

Provide compassionate, in-home nursing care to adult and geriatric patients across a community-based service area. Conduct comprehensive patient assessments, develop and coordinate individualized care plans, manage and monitor medications, and deliver skilled nursing interventions.
