Role Overview

• Competitive Compensation: $80,000 - $105,000 (+ mileage)
• Work schedule: Four 10-hour days (Wed - Sat, Thurs - Sun, or Fri - Mon)
• 1:1 nurse-to-patient ratios for personalized care

Responsibilities

• Provide home-based care to patients recovering from illnesses, injuries, and chronic conditions
• Collaborate with a multidisciplinary team for high-quality care
• Assess patients, document progress, and communicate with healthcare providers
• Educate patients and families on care plans, medications, therapies, diet, and exercise
• Coach families on home safety and care strategies
• Coordinate discharge from services ensuring care goals are met

Qualifications

• Graduate of an accredited Nursing Program with an active RN license in IL
• Minimum 2 years of nursing experience, preferably in home healthcare
• Current CPR and BLS certifications
• Knowledge of home health regulations
• Strong clinical judgment, communication skills, and compassion
