Boost Home Healthcare - Greater Boston is seeking a compassionate Registered Nurse to join our home-based care team as a Home Health Case Manager. You will build relationships with patients and families, ensuring the right care is delivered at the right time in the comfort of their homes.

As the primary contact for a caseload, you will complete OASIS assessments, develop care plans with the interdisciplinary team, coordinate services, and monitor outcomes.

Qualifications

What You'll Be Doing

• Manage the Care Journey: Serve as the primary point of contact for a caseload of home health patients, ensuring seamless coordination of services.
• Conduct Comprehensive Assessments: Complete OASIS assessments and develop individualized care plans in collaboration with the interdisciplinary team.
• Coordinate Services: Work with physicians, nurses, therapists, social workers, and home health aides to ensure timely delivery of care and optimal outcomes.
• Monitor and Evaluate: Track patient progress, update care plans, and communicate with families & caregivers as needed.
• Documentation: Maintain accurate and timely clinical documentation in accordance with Medicare guidelines and agency policy.
• Advocate for the Patient: Provide education, resources, and emotional support to patients and their families to promote independence & wellness.

What We’re Looking For

• License: RN (Registered Nurse) with active (state) license.
• Experience: At least 1 year of experience in home health or case management preferred.
• Skills: Strong critical thinking, communication, time management, and the ability to work independently.
• Tech-Savvy: Comfortable using EHR systems (experience with Kantime, Axxess, HCHB, or WellSky is a plus).
• Reliable Transportation: Valid driver’s license, auto insurance, and access to a dependable vehicle.

Pay per visit rates starting at $65
