Job summary

Addus HomeCare is seeking a registered nurse to deliver professional in-home nursing care under an established plan of care. You will assess and intervene using the nursing process, document findings, and coordinate with the care team to ensure high-quality patient outcomes.

The role emphasizes home visits, patient education, and adherence to state and federal regulations. Flexibility for on-call and after-hours visits is required.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a RN in the state of practice.
• Clinical practical nursing experience preferably in intensive care, pediatrics, rehabilitation or medical surgical nursing.
• Home health experience preferred
• Ability to make home visits and provide nursing care in the home environment
• Documentation appropriately, legibly and timely; strong interpersonal, verbal, and written communication skills.
• Must have reliable transportation and a valid driver’s license.

Responsibilities

• Assess, plan and intervene using the nursing process to meet the patient needs.
• Observe and report signs of change in condition and document in the clinical record.
• Administer treatments and medications in the plan of care.
• Teach patients/caregivers appropriate self-care techniques.
• Notify Case Manager, physicians, and families of changes in condition.
• Perform supervisory visits to evaluate competency and provide instructions.
• Maintain data collection and reporting systems to measure outcomes.
