Job summary

Elara Caring is seeking a Registered Nurse Home Health to provide care in patients' homes across Texas. You will assess patient needs, perform initial and ongoing assessments, and deliver nursing care per plan and physician orders to ensure high-quality outcomes.

The role requires an active RN license, 1+ year of clinical experience, and the ability to lift up to 50 pounds while traveling between patient homes.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associates or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Home Health experience is preferred, but not required.

Responsibilities

• Assessing patients to determine their care needs.
• Completing initial and ongoing patient assessments.
• Protecting patient and staff privacy in accordance with HIPAA.
• Providing nursing care according to the plan of care and physician orders.
• Monitoring patient conditions and reporting changes to the care team.
• Educating patients and families on their care and safety.
• Taking appropriate action when patient conditions change.
• Helping ensure quality, safe, and continuous patient care.
