Job summary

Advantage Home Health Services in Dillsburg, PA seeks a Home Health Registered Nurse to join our expanding field-based team. This full-time role focuses on delivering quality in-home patient care and coordinating with Case Managers to ensure optimal outcomes.

You will conduct Start of Care assessments, draft care plans, reconcile medications, maintain accurate documentation, and provide patient and family education per assessment and plan.

Qualifications

• Active RN license in the state of practice (PA).
• Home health nursing experience is a plus.

What You'll Do:

• Conducting Start of Care comprehensive assessments and completing OASIS timely
• Drafting care plans, reconciling patient medications, and ensuring orders for care are accurate and complete
• Preparing accurate, orderly, and timely documentation
• Coordinating care with patient and provide proper reports to Case Manager
• Provides health care instructions to the patient/family as appropriate per assessment and plan

About You

• An active RN license in the state in which practicing or New Grad with eligibility to take NCLEX.
