CARE FIRST HOME HEALTH CARE INC is seeking a Registered Nurse for home health care in California. The role involves planning, directing, and delivering home care services to improve patient quality of life.

Responsibilities include comprehensive assessments, creating and updating care plans, administering medications, supervising support staff, and coordinating with physicians. A CA RN license and CPR certification are required; a valid driver's license and auto insurance are also necessary.

Competenze

• Licensed Registered Nurse in the state.
• Current CPR certification.
• Valid driver’s license and car insurance.
• One year of nursing experience preferred in home health, hospice, or public health.
• Bachelor's degree in nursing is optional.

• Conducts initial comprehensive nursing assessment to determine home care needs.
• Develops and updates a care plan with measurable goals and time frames.
• Reassesses patient status and revises plan of care as needed.
• Administers medications and treatments per physician orders.
• Supervises LVNs and CHHAs per agency policies.
• Educates patient and family on medication management and disease prevention.
• Documents clinical notes accurately and timely.
