AdvisaCare in Michigan is seeking an experienced Registered Nurse (RN) Clinical Manager to lead our Home Health team and ensure high-quality patient outcomes. You will oversee clinical operations, support field clinicians, and promote regulatory compliance across Medicare guidelines.

The role requires strong leadership, 1–2 years of home health nursing, excellent communication, and the ability to travel within the service area to coach staff and drive performance improvement.

Qualifications

• Current, unrestricted RN license in Michigan.
• Graduate of an accredited nursing program.
• Minimum of 1–2 years of home health nursing experience.
• Knowledge of Medicare home health regulations and standards of practice.
• Excellent communication, leadership, and organizational skills.
• Valid drivers license, reliable transportation, and ability to travel within the service area.

Responsibilities

• Provide clinical leadership and supervision to home health staff, including RNs, LPNs, and aides.
• Ensure compliance with Medicare Conditions of Participation, accreditation standards, and agency policies.
• Review and monitor clinical documentation for accuracy, completeness, and regulatory compliance.
• Support the development and implementation of patient-centered care plans.
• Oversee OASIS accuracy, timeliness, and quality improvement initiatives.
• Assist with orientation, training, coaching, and performance management of clinical staff.
• Participate in quality assurance and performance improvement (QAPI) activities.
• Collaborate with physicians, referral sources, patients, families, and interdisciplinary team members.
• Monitor patient outcomes and identify opportunities for clinical excellence and operational efficiency.
• Assist with scheduling, staffing, and workflow management as needed.
• Serve as a clinical resource and mentor for field staff.

Position Summary

Preferred:

• Previous supervisory or management experience.
• Experience with EMR/EHR systems used in home health.
• Knowledge of quality improvement and utilization review processes.
• Familiarity with accreditation standards (CHAP, ACHC, or Joint Commission).
