Job summary

UnitedHealth Group is seeking a Registered Nurse in Home Health for Alabama HomeCare of Fort Payne to provide and direct nursing care in patients' homes as prescribed by physicians. You will coordinate care with the physician, agency staff, patients and families to maintain continuity of care.

Responsibilities include initiating and revising the plan of care, supervising home health aides and nurses, completing OASIS assessments, coordinating with clinicians, and ensuring compliance with laws,

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License and access to a dependable vehicle or reliable transportation.
• CPR certification preferred.

Responsibilities

• Develop, initiates, and revises the plan of care in collaboration with the physician and health care professionals.
• Supervise home health aides and licensed nurses; provide instruction and assign tasks.
• Complete comprehensive assessments (OASIS) and medication reconciliation; document visits per policy.
• Coordinate care with physicians, agency staff, patients, families, and community resources to ensure continuity of care.
