Job summary

UnitedHealth Group is seeking a Registered Nurse in Home Health to provide and coordinate nursing care in patients’ homes, ensuring compliance with laws, regulations, and agency policies. You will collaborate with physicians and the care team to maintain continuity of care and optimize patient outcomes.

The role involves supervising home health aides, conducting comprehensive assessments, and coordinating the plan of care with the patient, family, and community resources.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR certification (preferred or able to obtain within 90 days of hire).
• 1+ years of Home Health experience.
• Ability to work independently.
• Solid communication, writing, and organizational skills.

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed staff, provides instruction, and assigns tasks according to state and federal regulations.
• Provides required supervisory visits and ensures plan of care is followed.
