Ascension At Home is seeking a Home Health Registered Nurse (RN) to support patient recovery at home, promote independence, and coordinate care. You will educate patients and families on disease management while providing clinical oversight to an interdisciplinary team.

Michigan residents with an active RN license are encouraged to apply. Responsibilities include case management, driving to patient homes, and delivering high-quality, compassionate care in a home setting.

Qualifications

• RN license in the state you work
• Two years of nursing experience, home health preferred
• Current driver’s license
• CPR and BCLS certification required

Home Health Registered Nurse (RN) Requirements

• RN license in the state you work
• Two years of experience as a registered nurse, home health experience preferred
• Current driver’s license
