Job summary

Home Sweet Home Health Agency is seeking compassionate Home Health RNs to provide skilled nursing visits in Maricopa County, AZ. You will conduct in-home assessments, deliver wound care, manage medications, and develop care plans for geriatric and adult patients.

This PRN/part-time role offers the flexibility to schedule visits, with opportunities for per-visit compensation and professional collaboration with a supportive team.

Qualifications

• Current Arizona RN license in good standing.
• Graduate of an accredited nursing program; BSN preferred.
• Minimum of 1 year RN experience; home health preferred.
• Experience with geriatric patients and mobile/home visits.
• Valid driver’s license and reliable transportation to travel across East and South East Valley.
• Strong clinical assessment and communication skills.

Responsibilities

• Perform comprehensive nursing assessments in patients’ homes.
• Deliver skilled nursing care including wound care and medication management.
• Develop and update individualized home health care plans.
• Educate patients and caregivers on disease management and safety.
• Collaborate with physicians, case managers, therapists, and care teams.
• Complete accurate clinical documentation in compliance with standards.
• Monitor patient progress and communicate changes to the care team.

Home Health RN Responsibilities

As a Home Health Registered Nurse (RN), you will provide skilled nursing services throughout Maricopa County and surrounding Arizona communities. Responsibilities include:

• Conduct comprehensive nursing assessments in patients’ homes
• Deliver skilled nursing care including wound care, medication management, treatments, and patient monitoring
• Develop, implement, and update individualized home health care plans
• Educate patients and caregivers on disease management, medications, safety, and ongoing care needs
• Collaborate with physicians, case managers, therapists, and interdisciplinary care teams
• Complete accurate and timely clinical documentation in compliance with agency and regulatory standards
• Monitor patient progress and communicate changes in condition to the care team
• Support improved patient outcomes, independence, and quality of life

Qualifications

To be considered for this RN Home Health position in Arizona, candidates should meet the following requirements:

• Current Arizona Registered Nurse (RN) license in good standing
• Graduate of an accredited nursing program (BSN preferred)
• Minimum of 1 year of RN experience required
• Home Health nursing experience strongly preferred
• Experience working with geriatric patient populations preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and organizational abilities
• Valid driver’s license, reliable transportation, and ability to travel throughout the East and South East Valley area
• Ability to work independently while delivering compassionate mobile care
