Home Sweet Home Health Agency in Arizona is seeking compassionate registered nurses (RNs) to join our mobile home health team in Maricopa County. This PRN role focuses on delivering skilled nursing care in patients' homes, including wound care, meds management, and care planning.

Travel throughout East and South East Valley is required. We offer flexible scheduling, competitive per-visit pay, and strong clinical leadership to support independent practice in the field.

Competenze

• Current Arizona RN license in good standing.
• Graduate of an accredited nursing program; BSN preferred.
• Minimum of 1 year RN experience; home health experience preferred.
• Experience with geriatric patients preferred.
• Strong clinical assessment and critical thinking skills.
• Excellent communication and organizational abilities.
• Valid driver’s license and ability to travel throughout East & South East Valley.

Home Health RN Responsibilities

As a Home Health Registered Nurse (RN), you will provide skilled nursing services throughout Maricopa County and surrounding Arizona communities. Responsibilities include:

• Conduct comprehensive nursing assessments in patients’ homes
• Deliver skilled nursing care including wound care, medication management, treatments, and patient monitoring
• Develop, implement, and update individualized home health care plans
• Educate patients and caregivers on disease management, medications, safety, and ongoing care needs
• Collaborate with physicians, case managers, therapists, and interdisciplinary care teams
• Complete accurate and timely clinical documentation in compliance with agency and regulatory standards
• Monitor patient progress and communicate changes in condition to the care team
• Support improved patient outcomes, independence, and quality of life

Qualifications

To be considered for this RN Home Health position in Arizona, candidates should meet the following requirements:

• Current Arizona Registered Nurse (RN) license in good standing
• Graduate of an accredited nursing program (BSN preferred)
• Minimum of 1 year of RN experience required
• Home Health nursing experience strongly preferred
• Experience working with geriatric patient populations preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and organizational abilities
• Valid driver’s license, reliable transportation, and ability to travel throughout the East and South East Valley area
• Ability to work independently while delivering compassionate mobile care
