Job summary

JTC Home Health Inc. in Southern California seeks a dedicated Registered Nurse to join our home-based care team serving Temecula, Murrieta and Menifee.

The RN will plan, direct, and provide skilled nursing services in patients' homes, coordinating with physicians and an interdisciplinary team to ensure quality care. You will perform assessments, create care plans, manage medications, educate patients and families, and maintain meticulous records.

Qualifications

• Licensed as a Registered Nurse in California.
• Home health or community health education experience preferred.
• Ability to develop care plans and educate patients.

Responsibilities

• Conduct initial patient assessments and develop individualized care plans.
• Provide skilled nursing care, including wound care, medication management, and patient education.
• Monitor patient health and adjust care plans with physician collaboration.
• Educate patients and families on health management, disease prevention, and self-care techniques.
• Maintain accurate and timely patient records.

Job Description Summary:

As a Registered Nurse at JTC Home Health, you will be an integral part of our team, providing exceptional care to our patients in the comfort of their own homes. Your role will involve planning, organizing, and directing home care services. We are looking for an experienced nursing professional with a strong background in community health education. You will utilize community resources to plan and direct services that meet the unique needs of individuals and families.

Key Responsibilities:
