Job summary

Cruz Home Health Care LLC in Ontario, CA is seeking a compassionate Registered Nurse to join our home health team, delivering personalized, high-quality care in patients' homes.

This role requires a California RN license and at least 1 year of clinical nursing experience; home health experience is preferred. You will travel to Los Angeles, San Bernardino and Riverside Counties, coordinate with an interdisciplinary team, educate patients and families, and maintain accurate documentation.

Qualifications

• Active, unrestricted California RN license.
• Minimum 1 year of clinical nursing experience; home health preferred.
• Knowledge of OASIS documentation and Medicare/Medicaid home health regulations a plus.
• Valid California driver's license, reliable transportation, and auto insurance.
• Strong clinical assessment, critical thinking, and time management skills.
• Excellent communication and patient education abilities.
• Current CPR/BLS certification.
• Willing to travel to Los Angeles, San Bernardino, and Riverside Counties.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized care plans for home health patients.
• Administer medications, wound care, IV therapy, and other skilled nursing treatments in the home setting.
• Monitor and evaluate patient health status, documenting changes and reporting to physicians as needed.
• Educate patients and family members on disease management, medication adherence, and home safety.
• Coordinate care with interdisciplinary team members including physical therapists, social workers, and physicians.
• Maintain accurate and timely clinical documentation in compliance with Medicare, Medicaid, and agency standards.
• Ensure patient care aligns with OASIS assessments and home health plan of care requirements.
