Shifts: 12 Hour NOC

Location(s): La Jolla, North San Diego County

Hourly Pay Rate: $45-$80 based on experience

The RN collaborates with interdisciplinary team members to develop and implement individualized care plans and provides constant feedback to clients, client representatives, and nurses.

Responsibilities

• Complete assessment of client and needs
• Education to client and families
• Chronic Care Management
• Develop care plan and establish goals
• Post‑surgical and wound care as needed
• Wellness support

Qualifications

• Current, unrestricted California registered nurse license
• 3 years of experience as a registered nurse
• Home Health experience preferred
• Proof of negative TB result
• Physical screening documentation within the last year
• Active California driver’s license and car insurance
• Cell phone with internet access
