Job summary

Wheatland Health Community Care is seeking an experienced Home Health Registered Nurse to provide skilled in-home visits across our Florida service area. You will perform assessments, implement care plans, and educate patients and families to promote safety and independence.

The role offers salaried, full-time or part-time options with visits scheduled within standard business hours, and a per-visit incentive structure beyond the baseline productivity. Paid mileage is included as a perk.

Qualifications

• 3+ years of nursing experience required.
• Active license to practice as an RN in Florida.
• Reliable transportation for in-home visits.

Responsibilities

• Perform in-home skilled nursing visits: SOCs, recerts, routine visits, and discharges.
• Document and bill accurately in the EMR with assessments and care completed.
• Coordinate with physicians, therapists, and care coordinators to align orders.
• Monitor changes in condition and address safety risks to prevent hospitalizations.
• Support compliance with home health standards and QA activities.

What You'll Love

Status: Salaried W-2 position (Full-Time or Part-Time) - $1,500-$2,000 weekly

Productivity expectation of 30 patient visits per week, with an additional per-visit rate of approximately $65–$70 for visits completed above that threshold

Schedule: Visits fall within business hours (8:00 AM–8:00 PM); you won't work all 12 hours

What You’ll Do

• Perform in‑home skilled nursing visits - SOCs, recerts, routine skilled visits, discharges; complete thorough assessments, implement plan of care, and educate patients/families.
• Document accurately and on time in the EMR (assessments, care provided, orders, signatures) to support billing, compliance, and continuity.
• Coordinate care & communicate with physicians, therapists, care coordinators, and admin to align orders/authorizations and keep care plans current.
• Monitor progress & escalation concerns (changes in condition, safety risks, non‑compliance) to prevent adverse events and avoid unnecessary hospitalizations.
• Support compliance & quality - follow policies, payer requirements, and home health standards; participate in QA and care plan updates.

We're looking for a nursing professional who can meet the following qualifications:

• 3+ years' nursing experience
• Active Florida RN license
• Reliable transportation for in‑home visits

• Prior home health/field experience
• SOC/Recert/Discharge experience
• Strong documentation & OASIS
• Spanish speaking
