Job summary

Wheatland Health Community Care is seeking Home Care RNs to cover Osceola and Brevard counties in Florida. Roles are available as full-time or part-time positions, salaried W-2.

You will deliver skilled nursing at home and support independence for patients with compassion and professionalism. The position offers a weekly salary between $1,500 and $2,000, with a productivity expectation of 30 visits per week and additional pay for visits beyond that threshold.

Qualifications

• Active Florida RN license and reliable transportation.
• 3+ years of nursing experience.
• Preferred: home health or field-based experience.
• SOC/recert/discharge experience.
• Strong documentation & OASIS.
• Spanish a plus.

Responsibilities

• Map visits in chosen territory and perform SOCs, recerts, routine skilled visits, and discharges.
• Provide thorough assessments and teach patients and caregivers.
• Document EMR accurately to keep care billable and coordinated.
• Coordinate with physicians, therapists, care coordinators, and admin staff.
• Respond quickly to changes in condition or safety concerns.

Wheatland Health Community Care is hiring Home Care RNs across Osceola County and Brevard County, FL for Full‑Time or Part‑Time roles. You'll deliver skilled, compassionate care that protects independence at home-and you'll feel the rewards every single day.

Status: Salaried W-2 position (Full‑Time or Part‑Time) - $1,500-$2,000 weekly

Productivity expectation of 30 patient visits per week, with an additional per-visit rate of approximately $65–$70 for visits completed above that threshold.

You start you day as a Home Care RN by mapping your visits within your chosen territory, then step into homes to complete SOCs, recerts, routine skilled visits, and discharges-performing thorough assessments, implementing the plan of care, and teaching patients and caregivers in language that sticks; between visits, you capture accurate EMR documentation (assessments, care, orders, signatures) to keep care billable and coordinated; you update and align with physicians, therapists, care coordinators, and admin staff, responding quickly to changes in condition, safety concerns, or non‑compliance to prevent avoidable ED trips; you round out your day by reviewing QA insights and ensuring policies and payer rules are met so your patients keep progressing at home.

WORK SCHEDULE FOR OUR HOME CARE RN

• Full‑Time or Part‑Time with visits scheduled within 8:00 AM–8:00 PM; you won't work a 12‑hour shift
• Choose at least one territory and cover all ZIP codes in that territory (you may choose multiple)

Must have the ability to pass a background check and drug screening test.
