Job summary

Optimal Care seeks a compassionate Home Health Registered Nurse to provide skilled nursing care to patients in their homes. You will perform comprehensive assessments, develop and update care plans, administer medications and treatments, and educate patients and families on disease management and safety.

You will coordinate with physicians, therapists, and caregivers, document in our electronic health record, and manage a mobile caseload with reliability and strong clinical judgment.

Qualifications

• Licensed RN with current state license.
• Skilled in comprehensive patient assessments and care planning.
• Strong clinical judgment and excellent communication are essential.

Responsibilities

• Perform comprehensive home health nursing assessments.
