Job summary
Optimal Care is seeking a compassionate Home Health Registered Nurse to provide skilled nursing care to patients in their homes. You will perform comprehensive assessments, develop and update care plans, administer medications and treatments, and educate patients and families on disease management and safety.
You will coordinate with physicians, therapists, and caregivers, document in our electronic health record, and manage a mobile caseload.
Qualifications
- Licensed as a Registered Nurse (RN) in the relevant state.
- Ability to perform comprehensive home health assessments and develop care plans.
- Administer medications and treatments as prescribed.
- Educate patients and families on disease management and safety.
- Coordinate with physicians, therapists, and caregivers; document in EHR.
- Manage a mobile caseload with reliable transportation.
