Optimal Care is seeking a compassionate Home Health Registered Nurse to provide skilled nursing care to patients in their homes. You will perform comprehensive assessments, develop and update care plans, administer medications, and educate patients and families on disease management and safety.

You will coordinate with physicians, therapists, and caregivers, document in our electronic health record, and manage a mobile caseload while delivering high-quality, patient-centered care.

Qualifications

• Licensed RN required
• Home health nursing experience desirable
