Job summary

Humana, through CenterWell Home Health, seeks a Home Health Physical Therapist Assistant to deliver in-home care, support functional goals, and partner with a Physical Therapist and care team to enhance patient independence.

Responsibilities include administering PT treatment, documenting records, attending meetings, and ensuring safety with infection control principles. Requires state PT license, driver’s license, CPR, and geriatric knowledge.

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Comprehensive knowledge of geriatric physical, emotional, social, and biological changes.
• Read, write, and speak fluent English.

Responsibilities

• Administer the physical therapy treatment plan to patients in their homes.
• Perform non-complex active and passive manual therapies and modalities.
• Document client records and submit documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify Physical Therapist or Branch Director of equipment issues or staff changes.
• Apply infection control and universal precautions in patient care.
