Job summary

Humana’s CenterWell Home Health is seeking a dedicated Physical Therapy Assistant to administer home-based PT plans under supervision. You will work in patients’ homes using non-complex therapeutic modalities and document care in a timely manner.

The role emphasizes collaboration with a Physical Therapist, adherence to infection control, and ongoing coordination with the department. A valid PTA license, CPR, and English proficiency are required.

Qualifications

• Active Physical Therapy Assistant license or eligible license in the state of practice.
• CPR certification is required.
• Experience collaborating with a healthcare team.
• Fluent reading, writing, and speaking of English.
• Knowledge of geriatric patient needs and changes.

Responsibilities

• Administer the physical therapy treatment plan to patients, in their homes, as developed by the Physical Therapist.
• Perform non-complex manual therapies and modalities.
• Document client records and submit weekly to office.
• Attend department meetings and coordinate in-services.
• Report equipment or supply issues to Physical Therapist or Branch Director.
• Apply infection control and universal precautions in patient care.
