Job summary

CenterWell Home Health is seeking a licensed Physical Therapist Assistant to deliver in-home therapy under a supervising Physical Therapist. You will implement treatment plans, document care, and collaborate with a multidisciplinary team to promote functional independence for geriatric patients.

This full-time role includes a 40-hour work week, driving to patient homes, and competitive pay per visit with benefits including medical, dental, vision, and a 401(k).

Qualifications

• Current state license to practice Physical Therapy.
• Valid driver's license and reliable transportation.
• Current CPR certification.
• Experience collaborating with a healthcare team.
• Comprehensive knowledge of geriatric changes (physical, emotional, social, biological).
• Read, write, and speak fluent English.

Responsibilities

• Administer PT treatment plan to patients in their homes.
• Perform non-complex manual therapy, exercises, and modalities as prescribed.
• Document client records and submit on time.
• Attend educational meetings and coordinate department in-services.
• Notify PT or Branch Director of equipment or staff changes.
• Apply infection control and universal precautions.
