Job summary

CenterWell Home Health seeks a licensed Physical Therapist to deliver in-home therapy, assessing and advancing functional goals for geriatric patients.

Reporting to the Clinical Manager, you will collaborate with a multidisciplinary team, document care, and ensure infection control and safety protocols while helping patients regain independence.

Qualifications

• Current state license to practice Physical Therapy
• Valid CPR certification
• Experience collaborating with a team of healthcare professionals
• Comprehensive knowledge of geriatric population changes
• Read, write, and speak fluent English

Responsibilities

• Administer the physical therapy treatment plan to patients, working onsite in their homes
• Administer non-complex manual therapeutic exercises and modalities as prescribed
• Document client records and submit clinical documentation on time each week
• Attend educational meetings and coordinate department in-services
• Notify supervisor of equipment issues, supply shortages, or client changes
• Apply infection control and universal precautions in patient care
