CenterWell Home Health is hiring a Home Health Physical Therapist Assistant to work in patients' homes under the supervision of a Physical Therapist. You will deliver the prescribed plan, perform therapeutic exercises and document progress for weekly review.

The role requires a current PT license, a valid driver's license, CPR and strong teamwork in geriatric care. English fluency is essential to communicate with patients and care teams.

Qualifikationen

• Current state license to practice Physical Therapy.
• Valid driver's license, auto insurance and reliable transportation.
• Current CPR certification.
• Experience collaborating with a team of healthcare professionals.
• Comprehensive knowledge of geriatric changes.
• Read, write, and speak fluent English.

Aufgaben

• Administer the physical therapy treatment plan to patients in their homes.
• Perform non-complex manual therapies and modalities as directed.
• Document client records and submit clinical documentation weekly.
• Attend educational meetings and coordinate department in-services.
• Notify supervisor of equipment failures, supply issues or changes.
• Apply infection control and universal precautions in patient care.
